Thank you for your interest in our Assisted Living Manager Training Program! To begin, all applicants must complete the program registration form. After registering, we will proceed with enrollment, which includes submitting the required documents and completing any pre-assessment steps.

The total tuition for the Assisted Living Manager Program is $499.00, which includes:

• 40 hours of online coursework

• 8-hour live Q&A session

• Training materials to prepare you for the Arizona state exam

Required documents for enrollment:

• Valid U.S. photo ID

• Social Security card or “NO SS” affidavit

Please note: Students are responsible for all fees associated with D&S Diversified Technologies (Headmaster) and any applicable Arizona state licensing or testing fees. We will guide you through the registration process with the testing provider once your training is complete.

1. Should have been certified as a manager in all levels of care services

2. Must be physically fit

3. Students must be over 21 years of age

4. High School diploma or GED is recommended

5. 7th grade reading level recommended

6. Must possess a valid photo ID

7. Social Security Card

8. No criminal conviction within the last 5 years

9. Negative TB skin test or chest x-ray

10. Must have filed for a DPS fingerprint clearance card and should possess the card by the time of lab instruction.

HOLDER IN DUE COURSE: I acknowledge that upon signing this admission agreement, paying the tuition fee, and successfully completing the course, I will rightfully receive the Certificate of Completion from D & S Diversified upon passing the final examination. I understand that I will be the legitimate owner of this certificate, absolved from any further obligations to the school or any other parties. STUDENT ACKNOWLEDGMENTS: Students will initial after every statement. 1. I have carefully read and received an exact copy of this enrollment agreement. 2. I understand that the school may cancel my enrollment if I fail to comply with attendance, academic, and financial requirements or I disrupt the normal activities of the school. 3. I also understand that this institution does not guarantee job placement to graduates upon program/course completion or upon graduation.

Below are the required items needed to take the state exam. If you do not currently have all the items, you will need at least your ID/Driver's License and Social Security Card to enroll in our program.
